Patient’s Medication Errors

(Reuters Health) - Half of all heart patients made at least one medication-related mistake after leaving the hospital, and guidance from a pharmacist didn't seem to reduce those errors, in a new study.

Consequences of mistakes - such as forgetting to take certain drugs or taking the wrong dose - can range from side effects like constipation to more serious drops in blood pressure. Two percent of errors were life-threatening.

Hospitals involved in the study were already taking steps to prevent medication mistakes in addition to the extra pharmacist intervention, said Dr. Sunil Kripalani, the study's lead author from the Vanderbilt University Medical Center in Nashville, Tennessee.

"We were surprised to see that in spite of these efforts that 50 percent (of patients) were still having these medication errors," he told Reuters Health.

Although the pharmacist visits didn't help the average patient, he added, certain ones seemed to benefit - such as patients who were on multiple drugs or had trouble understanding health information.

As for traditionally lower-risk patients, he said other strategies to prevent errors may be needed.

Explanation Text, Tsunami

☽༓・*˚⁺‧͙ well hello there! Welcome back with me,
khazimah nur, from 12 social 1 ‧͙⁺˚*・༓☾

Today, I'm going to tell you an Explanation Text, the title is Tsunami! without any further ado, let's get it! :D

:cloud: ·̩͙✧ Definition
Explanation text is a text that explains about a process or sequence of phenomena.
There are 2 types of explanation text; sequential and cause and effect.

:cloud: ·̩͙✧ Purposes

An explanation text is written to explain how and why something (natural or socio-cultural phenomena) in the world happen. The text contains the process or sequence of the phenomena.

:cloud: ·̩͙✧ Generic Structure
The generic structures of explanation text consists of three components.
Title
General statement -> Stating the phenomena or issues which are being explained.
Explanation -> Stating the series of steps which explain the issues or phenomena.

:cloud: ·̩͙✧ Language Features
  1. Focusing generic participant -> an object or phenomenon which is being discussed, such as; rain, chocolate, etc.
  2. Using present tense -> This kind of text mostly applies present tense for its sentences, such as; "rain is the primary source of fresh water for most areas of the world, ......."; Chocolate starts from a tree called cacao tree, and etc.
  3. Using chronological connectors -> The text applies chronological connectors / chronological connections / connectives, such as; first, second, third, then, after, after that, finally, so, as a consequence, and etc.
  4. Using passive voice -> The text sometimes applies passive form, such as; "... the beans are fermented for about a week, dried under the sun, and shipped to the chocolate maker...".
  5. Using noun, pronoun, verb, and etc. -> Like so many other text, explanation text absolutely applies a lot of nouns, pronouns (subject pronoun, object pronoun, possessive pronoun, demonstrative pronoun), action verb, and etc.

:cloud: ·̩͙✧ Example

Tsunami

We all know that tsunamis are a series of powerful and destructive waves. Tsunami is the deadliest wave because the wave speed is incredibly fast. But do you know how a tsunami happens?

Tsunami originated from Japan, “tsu’ meaning harbor and “nami” meaning wave. They are normally caused by earthquakes, landslides, or volcanic eruptions in, or close to an ocean. After some sort of underwater disturbance, this causes energy to rise up to sea level pushing the water high into the air. Gravity kicks in and pulls it down again and then consequently causing the water to filter out into many different directions.

When a tsunami begins to approach the shore the energy in the wave is compressed. As a result, the water is pushed up into the wave, resulting in a powerful tsunami. If the trough hits the shoreline first, it then causes the tide to drastically retreat.

So, tsunamis happen because of natural disasters such as earthquakes or volcanic eruptions. People who live near the shoreline have to be careful because they are threatened by tsunamis whenever there is an earthquake.

Before we are going to smoke, it is better to look at the fact. About 50 thousands people die every year in Britain as direct result of smoking. This is seven times as many as die in road accidents. Nearly a quarter of smokers die because of diseases caused by smoking.
Ninety percent of lung cancers are caused by smoking. If we smoke five cigarettes a day, we are six times more likely to die of lung cancer than a non smoker. If we smoke twenty cigarettes a day, the risk is nineteen greater. Ninety five percent of people who suffer of bronchitis are people who are smoking. Smokers are two and half times more likely to die of heart disease than non smokers.
Additionally, children of smoker are more likely to develop bronchitis and pneumonia. In one hour in smoky room, non smoker breathes as much as substance causing cancer as if he had smoked fifteen cigarettes.
Smoking is really good for tobacco companies because they do make much money from smoking habit. Smoking however is not good for every body else.

:cloud: ·̩͙✧Example

 The Ugly Duckling

One upon time, a mother duck sat on her eggs. She felt tired of sitting on them. She just wished the eggs would break out.

Several days later, she got her wish. The eggs cracked and some cute little ducklings appeared. "Peep, peep" the little ducklings cried. "Quack, quack" their mother greeted in return.

However the largest egg had not cracked. The mother duck sat on it for several days. Finally, it cracked and a huge ugly duckling waddled out. The mother duck looked at him in surprise. He was so big and very gray. He didn't look like the others at all. He was like a turkey.

When the mother duck brought the children to the pond for their first swimming lesson., the huge grey duckling splashed and paddled about just as nicely as the other ducklings did. "That is not a turkey chick. He is my very own son and quite handsome" the mother said proudly.

However, the other animals didn't agree. They hissed and made fun of him day by day. Even his own sisters and brothers were very unkind. "You are very ugly" they quacked.

The little poor duckling was very unhappy. "I wish I looked like them" he thought to himself. One day, the ugly duckling run away and hid in the bushes. The sad duckling lived alone through the cold and snow winter. Finally the spring flowers began to bloom. While he was swimming in the pond, he saw three large white swans swimming toward him. "Oh, dear. these beautiful birds will laugh and peck me too" he said to himself. But the swans did not attack him. Instead, they swam around him and stroked him with their bills. As the ugly duckling bent his neck to speak to them, he saw his reflection in the water. He could not believe his eyes. "I am not an ugly duckling but a beautiful swam" he exclaimed.

He was very happy. From that day on, he swam and played with his new friends and was happier than he had never been.
